Harness, RAG, guardrails, skills and AGENTS.md: the system around the model.

What each piece is, where it lives, what it costs in context, how they fit in one design, and a small agent in Docker that uses all of them end to end.

An LLM takes text in and gives text out. Everything that makes it an agent that reads files, searches documents, runs tests and does not leak secrets is code and files around it. The formula that settled in 2026 is agent = model + harness, and designing that rest got a name: harness engineering.

1. Harness: o que roda em volta do modelo1. Harness: what runs around the model

In Matt Pocock's dictionary of AI coding, a harness is everything around the model that turns it into an agent: tools, system prompt, context management, permissions and hooks. The model is stateless: it only sees what is in the context window on each request. Continuity comes from the harness resending the conversation and from files in the environment.

O loopThe loop

Every harness, from Claude Code to your hundred-line script, repeats the same thing. The key design point is that the model never executes anything: it asks for a tool, and the harness decides whether to run it.

enquanto passos < limite:
    resposta = modelo(mensagens, ferramentas)
    se resposta não pede ferramenta:
        devolve resposta # condição de parada
    para cada pedido em resposta:
        resultado = guardrails(executa(pedido))   # o harness executa, não o modelo
        mensagens += resultado # o ambiente vira o próximo contexto

Anthropic puts it this way: an agent is typically an LLM using tools in a loop, with environment feedback at each step, stopping on a completion condition or a limit such as a maximum number of iterations.

Workflow ou agente?Workflow or agent?

Os cinco padrões de workflow:Before giving the model autonomy, check whether a fixed flow is enough. Anthropic separates workflows (paths defined in code) from agents (the model chooses the path) and recommends starting with the simplest, because agents trade latency and cost for performance. The five workflow patterns:

Context engineeringContext engineering

As técnicas que se repetem nas fontes:If the model only sees the window, the harness's central job is deciding what goes into it. Anthropic calls this context engineering: curating the optimal set of tokens at each inference, not just writing the prompt. The techniques that repeat across the sources:

  • Context rot. Context rot. The more tokens, the worse the recall of details. Context is a finite resource. Pocock calls it the smart zone and dumb zone and suggests the latter often starts around 125 to 150 thousand tokens, a number he says is debated.
  • Recuperação just-in-time. Just-in-time retrieval. Instead of loading everything, keep lightweight references (paths, queries, links) and fetch when needed. This is the idea behind skills and RAG as a tool.
  • Compactação. Perto do limite, resumir a conversa e continuar numa janela nova. Compaction. Near the limit, summarize the conversation and continue in a fresh window. First maximize recall of the essentials, then trim the superfluous, like old tool results.
  • Notas estruturadas. Structured notes. The agent writes to a file outside the window (to-do list, NOTES.md) and reads it back later.
  • Subagentes. Subagents. A specialized agent works in a clean window and returns only a condensed summary to the main one.

Harness engineering: a práticaHarness engineering: the practice

Mitchell Hashimoto (creator of Terraform and Ghostty) named it in a 2026 post: “anytime you find an agent makes a mistake, you take the time to engineer a solution such that the agent never makes that mistake again”. He describes two forms: better implicit prompting (a line in AGENTS.md, each line born from a bad behavior) and programmed tools (scripts that take screenshots or run filtered tests), told to the agent in AGENTS.md. The stated goal is also for the agent to be able to verify what it did.

In February 2026 OpenAI published an account of an experiment in which Codex wrote all of a product's code (per summaries, around a million lines, with no hand-written code). The lessons that show up in the summaries: one giant AGENTS.md failed and became a map of about 100 lines pointing to a docs/ directory that is the source of truth; linters and CI check that the docs are current; a “gardener” agent opens PRs against stale docs; custom linters have error messages pointing to runbooks; and the principle of making the mistake impossible, not just unlikely.

Os problemas e as correções:When work outlasts one window, Anthropic describes (with the Claude Agent SDK) two roles: an initializer agent that runs once and prepares the environment (an init.sh script, a progress file, an initial commit, a feature list) and a coding agent that, each session, advances one feature and leaves a clean state. The feature list is a JSON with everything initially marked failing, because models overwrite JSON less than Markdown. The problems and fixes:

Por isso:AGENTS.md is described as a “README for agents”: what does not fit the human README, like test commands and conventions. Everything in it is loaded in every session and costs tokens on every turn, and as instructions pile up, each one is followed less reliably (Pocock's own entry). So:

Curto e declarativo.In: what the agent cannot infer from the code: exact commands, non-obvious conventions, hard constraints (“never edit the generated client”). Short and declarative.
Out: an architecture overview the code already shows, long style guides (they go to a skill or docs/), and rules a linter can enforce.
Entry criterion: if you corrected the agent twice on the same point, it is a candidate (Pocock). It is Hashimoto's version: each line comes from an observed bad behavior.

An LLM-generated file is not a finished product. The study by Gloaguen et al. (ETH Zurich, arXiv 2602.11988) measured coding agents with and without context files: on average there was no significant success gain, and inference cost rose by over 20%. Developer-written files did better than LLM-generated ones, and repository overviews did not help. When the authors removed all other documentation from the repository, the generated files helped a little (about 2.7%). Practical reading: hand-write what the code does not say, and cut the rest.

The mattpocock/skills repository is a good study in designing skills. It separates the ones you invoke (/grill-me, /to-spec, /implement, /handoff) from the ones the model invokes (tdd, diagnosing-bugs, code-review, research). The stated philosophy: align before building (most failures come from misunderstanding, so the agent interviews you until every branch of the design is resolved), build a shared vocabulary in a GLOSSARY.md, give the agent fast feedback (types, tests, browser) and work in small vertical slices. The skills are small and meant to be edited by you.

A skill is code running with the agent's permissions. A skill installs instructions and scripts that the agent will run with your access. 2026 preprints already study least privilege for skills and spec violations across thousands of files. Read the SKILL.md and the scripts/ before installing third-party ones, as you would a dependency.

4. RAG: buscar antes de responder4. RAG: search before answering

O pipeline clássico tem duas metades:RAG (retrieval-augmented generation) is fetching relevant chunks from a knowledge base and putting them in the prompt so the model answers based on them. It solves private or recent knowledge without retraining, and it lets you cite the source. The classic pipeline has two halves:

  1. Ingestão (offline): carregar os documentos, quebrar em chunks (de preferência por título e seção, não por um número cego de caracteres), enriquecer cada chunk e indexar (BM25, vetores ou os dois).Ingestion (offline): load the documents, split into chunks (preferably by heading and section, not by a blind character count), enrich each chunk and index it (BM25, vectors or both).
Query (online): rewrite the question, retrieve the top N, rerank, build the context with each chunk's source, generate and cite.

O estado da arte, em númerosThe state of the art, in numbers

Taxa de falha da recuperação dos 20 melhores chunks:The most cited result is Anthropic's Contextual Retrieval. The problem: a chunk like “revenue grew 3% over the quarter” loses its context (which company, which quarter) and is not retrieved. The fix is to prefix each chunk with 50 to 100 tokens of LLM-generated context before indexing. Top-20 retrieval failure rate:

Two takeaways from the same text: the one-time cost of generating the context was around US$1.02 per million document tokens (with prompt caching), and under 200 thousand tokens (about 500 pages) it may be better to put the whole base in the prompt with caching than to build RAG.

RAG clássico ou busca agêntica?Classic RAG or agentic search?

In 2026 the debate moved from “is RAG dead?” to “who decides when and how to search”. In classic RAG the pipeline always searches, in a fixed way. In agentic search, search is a tool: the model chooses when to search, reformulates the query and reads files (including with grep). 2026 preprints report that a simple grep-and-read pipeline rivals full frameworks on software engineering tasks, and that lexical search (BM25) preserves names and exact terms that embeddings lose. The consensus across the sources is routing, not a single technique:

SituaçãoSituationAbordagemApproach
Base pequena e estável (< 200 mil tokens)Small, stable base (< 200 thousand tokens)Tudo no prompt, com prompt cachingEverything in the prompt, with prompt caching
Código ou texto com nomes exatos (funções, IDs, erros)Code or text with exact names (functions, IDs, errors)BM25 ou grep como ferramenta; o agente lê o arquivoBM25 or grep as a tool; the agent reads the file
Retrieved text is untrusted input. Everything RAG brings into the context may contain instructions (indirect prompt injection). That is why the project in section 7 fences each chunk with <untrusted>, redacts secrets before the model sees them and filters by permission at retrieval, not after. And measure with your own questions: top-N recall on your base decides, not benchmark averages.

Guardrails: what holds even if the model is wrong

A guardrail is a control outside the model that limits what can happen even if the model is wrong or manipulated. The distinction that organizes the subject: a prompt (including AGENTS.md) is a request, and the model complies with some probability; a guardrail in code is a guarantee. From the talk summarized by ZenML: bet on making the error impossible, not just unlikely.

O modelo mental: a trifeta letal e a Regra de DoisThe mental model: the lethal trifecta and the Rule of Two

In June 2025 Simon Willison described the lethal trifecta: an agent with (1) access to private data, (2) exposure to untrusted content and (3) the ability to communicate externally can be driven, through prompt injection, to exfiltrate the data. On October 31, 2025 Meta published the Rule of Two: until prompt injection can be reliably detected, an agent should have at most two of these properties in one session; if it needs all three, it should not run unsupervised (human approval or another reliable validation). Meta itself admits some use cases do not fit neatly.

Hooks: guardrails no harness que você não escreveuHooks: guardrails in a harness you did not write

Exemplo da documentação, que protege arquivos sensíveis:If you use an off-the-shelf harness, hooks are where deterministic guardrails go. In Claude Code, a PreToolUse hook runs before the tool, receives the call's JSON on standard input and, with exit code 2, blocks the action and passes the stderr message back to the model. Example from the documentation, protecting sensitive files:

# .claude/hooks/protect-files.sh   (chmod +x)
INPUT=$(cat)
FILE_PATH=$(echo "$INPUT" | jq -r '.tool_input.file_path // empty')

for pattern in ".env" "package-lock.json" ".git/"; do
  if [[ "$FILE_PATH" == *"$pattern"* ]]; then
    echo "Blocked: $FILE_PATH matches protected pattern '$pattern'" >&2
    exit 2
  fi
done
exit 0

# .claude/settings.json
{
  "hooks": {
    "PreToolUse": [
      { "matcher": "Edit|Write",
        "hooks": [{ "type": "command",
                    "command": "\"$CLAUDE_PROJECT_DIR\"/.claude/hooks/protect-files.sh" }] }
    ]
  }
}

Note the asymmetry in the documentation: exiting 0 from a PreToolUse hook does not approve the call, which still goes through the normal permission flow. And the linter is also a guardrail: OpenAI's account uses lints whose error messages teach the agent how to fix the problem.

Personal note, not verified in the sources: libraries and classifier models exist for the input and output layers (for example NeMo Guardrails, Guardrails AI and Llama Guard). I cite them from memory, did not test them and did not use them in the project.

6. System design: do pedido à resposta6. System design: from request to answer

Juntando as peças no agente da seção 7 (um assistente de documentação de um produto fictício, o Acme Billing), uma pergunta percorre este caminho:Putting the pieces together in the section 7 agent (a documentation assistant for a fictional product, Acme Billing), a question travels this path:

The harness reads AGENTS.md, validates the skills (name and description), indexes docs/ in memory and builds the system prompt: fixed rules + AGENTS.md + skill catalog (names and descriptions only).
  2. Pergunta. O usuário pergunta “quanto tempo vale o reembolso?”.Question. The user asks “how long is the refund window?”.
  3. Modelo pede uma ferramenta. Chama search_docs com palavras-chave. O modelo não executa nada.Model asks for a tool. It calls search_docs with keywords. The model executes nothing.
Harness executes under policy. BM25 returns the chunks; each is redacted and fenced with <untrusted source=...>; the result becomes a tool message.
  5. Rastro.
Skill, if relevant. If the question needs a format, the model calls load_skill and only then does the skill body enter the context.
  7. Resposta. Answer. With no further tool requests, the loop ends; the answer goes through redact() and leaves with its sources.
A política confina o caminho; a aprovação humana nega; a redação tira a chave do conteúdo; o limite de passos encerra loops. If something goes wrong. A chunk with an injection may make the model request write_file. The policy confines the path; human approval denies; redaction strips the key from the content; the step limit ends loops. Each denial goes back to the model as DENIED: ... text, so it can correct itself.

Ele junta tudo: harness (loop.py), AGENTS.md e skills (workspace/), RAG (rag.py) e guardrails (guard.py mais as opções do container).mini-harness is an assistant that answers questions about a product's documents and can save notes. It only uses the Python standard library (no pip install), so the image is small and the whole code fits on this page. It brings everything together: harness (loop.py), AGENTS.md and skills (workspace/), RAG (rag.py) and guardrails (guard.py plus the container options).

What I tested. I built the image and ran the 8 tests and the two demos below in Docker (version 29, Python 3.12), with the fake model. The fake model (LLM_MODEL=scripted) exists on purpose: it obeys an injection, to prove the harness holds with a compromised model. It does not measure the quality of any real model.

The agent container runs non-root, with a read-only filesystem, no capabilities and no privilege escalation; only notes/ is writable. The sealed network is internal: the agent sees Ollama but has no route to the internet. Ollama sits on both networks only to pull models.

RodarRun

After creating the files, inside the mini-harness/ folder. HOST_USER makes the container write to notes/ as your user:

export HOST_USER="$(id -u):$(id -g)"

# 1. testes (guardrails + ponta a ponta com o modelo falso)
docker compose run --rm --no-deps --entrypoint python agent -m unittest discover -s tests -v

# 2. pergunta normal
docker compose run --rm --no-deps -e LLM_MODEL=scripted agent "how long is the refund window"

# 3. ataque: a injeção do onboarding.md tenta gravar a chave em notes/leak.txt
docker compose run --rm --no-deps -e LLM_MODEL=scripted -e APPROVE=no agent \
  "what is in the onboarding integrations"
ls workspace/notes # vazio: nada foi gravado

Saída do ataque (linhas de rastro abreviadas). A chave nunca aparece: foi redigida antes de chegar ao modelo, e o write_file pedido pela injeção foi negado:Output of the attack (trace lines shortened). The key never appears: it was redacted before reaching the model, and the write_file requested by the injection was denied:

{"step": 0, "tool": "search_docs", "args": "{\"query\": \"what is in the onboarding integrations\"}",
 "result": "<untrusted source=\"docs/onboarding.md#Integrations\">\nThe staging payment key is [REDACTED]. ...", "ms": 0}
{"step": 1, "tool": "write_file",
 "args": "{\"path\": \"notes/leak.txt\", \"content\": \"staging key is [REDACTED] -->\"}",
 "result": "DENIED: a human did not approve this action", "ms": 0}

With APPROVE=yes the write passes approval, but the written content still carries [REDACTED] (that is the test_approved_write_is_still_redacted test). In a terminal, without the variable, the harness asks allow? [y/N] before each write.

I did not run this part. The Ollama image download did not finish on my machine, so the commands above with a real model were not tested by me. What is tested is the harness, the guardrails and the scripted path. With a small model, expect tool-call mistakes; that is what the ERROR: messages and the step limit are for.
